Once you follow the three steps, everything is fine.

1. I apply for permission to import lithium batteries to DHL. - Completed.
Today I have applied it. They told that it would take about one week.
As they would not accept the transportation unless the registration is completed, it would take some time.

2. Put one piece of attached label onto the carton.
Print out the label which is attached, and put it in a size 120x100mm or larger.

3. Prepare MSDS.
When you make shipment, I heard that you would be requested to submit MSDS.
Do you have the MSDS for lithium batteries?

I've found following bugs.
1. The vote up button is not displayed. If you put the cursor over, it is displayed.
2. When you change Slug, you'll see "404 Error". The Slug which is set up here is not used in anywhere.
3. The picture size(the height x the width) of the posted picture is enlarged and displayed. Would it be possible to display the picture in exact same size as the actual picture?
