The rule of SMART is an acronym as follows.

Specific targets like "I will do push-up everyday."
Measurable targets like "I will do it ten times."
Acceptable targets for himself/herself. The target is not fixed by others.
Realistic targets like 10 times. 100 times is not realistic.
Time limits of the target are specified. Just like "in today" etc.

In the rule of SMART, it is especially important that targets must be expressed with concrete numbers.
In addition, the target must be the one you will do, not the one you will have somebody do.
Using the rule of SMART, let's start talking with numbers and setting time limits.

So things that indicate what to do for solving problems are tasks, I think.
Specific examples are as follows.

a problem: Profit of the company is low.
a task: We have to have our profit increase.
a provision: Making sales higher, costs lower.

A difference between problems and tasks.
problems: It means the conditions and situations like "We cannot." and "We have something bad."
tasks: Themes we decided to do it.

Hurdles for achieving the goals and tasks: What you can conceive what to do for achieving the goals.

自分の事業場（会社）の安全衛生管理活動の現状を確認し、その現状に合わせて取り組むべき事項を決め、無理することなく、できるところから構築していくことが大切。
また、唯一の国際的な基準としてILO（国際労働機関）においてもOSHMSに関するガイドラインが策定されている。
OSHMSの構築に当たっては、厚生労働省の指針やILOのガイドラインは原則となるものをまとめたものです。

Narrow down the reasons of origination and spillage to around two each.

Were there any oversights or clumsiness in management of human resource, systems and procedures.
Each processes of "why so" will proceed individually when there are 2 or more options of "why so".
Conceive and create procedures until company can cope with it concretely [not just an idealistically] with 5 or 6 concepts of "why so".

この事実を知ったときは何も手につかず、何も考えることができなかった。

When I knew this fact, I could not do and think of anything.

対向車線からきたトラックが居眠り運転により、N君の車に追突してきたそうです。
その知らせを受けた時は全く信じられず、冗談だと思った程です。火葬の時には涙が止まらず、様々なことが思い出されました。お葬式には追突してきたトラックの会社の人も参列しました。さすがに事故を起こした人は来ませんでしたが、僕らはお葬式の最中でさえ、殴りかかろうかというほど怒りました。たった20年しか生きることが出来ず、人生これからというときに殺されてしまう悲しみは今でも忘れません。

I heard that a truck driven by a sleeping driver ran into N's car from the opposite lane.
When I heard the news, It was so unbelievable that I thought it was kidding. I could not stop my tear running at the cremation and many things were drifted in my mind. Staffs of the company which employs the driver attended the funeral. Though the man who drove did not appear as I thought, I got so angry that I almost hit them even during the funeral. I still remember the sadness that he was killed at most hopeful era of his life and he could not live more than only 20 years.
