I am writing to you today as I have an apology and a request to communicate to you. I have shipped out an item to you a few days ago, but I have discovered that the item that was shipped was not the item you have ordered. I deeply apologize for this.

I will immediately arrange for the item you have ordered, so my request is that you do not accept the item that was mistakenly shipped to you. I ask that you please decline to accept the item.
