I also acknowledge that you shall not be liable for any physical anomaly which may occur to my attendant to whom I recommend to eat.
I am over 18 years of age in perfect health condition and I have no injury or sickness but loves to eat spicy foods, so I order this menu based upon my own judgment that I am eligible to order the said food.

I also admit there is no responsibility for your shop if any of problem will be occurred to accompanied person if he or she will eat because of my recommendation .

I will place my order because I am confident in qualifying to challenge due not to have any illness for necessary of going to see doctor at the moment.
If you are accompanying children, then splashing the sauce of curry or responsibility for mis-feeding the children are because of me(including accompanying other adult as well).
I can admit that I am not complained about if food is too hot to eat and promise to never complain by myself.
The name undersigned was no doubt my name.
I shouldn't repeat but I am saying that your facilities don't have any responsibility.
