料理名の通り、注文したことも食べたこともすべては私の判断で自己責任であり、身体にいかなる異常が起きても貴店には一切責任がないことを認めます

私の同伴者にすすめ、同伴者が食し、その同伴者の身体にいかなる異常が起きても貴店には一切責任がないことを認めます

私は18歳以上の健康体そのもので、けがや病気をしていない辛い物好きですので、この商品を注文する資格があると自身で判断し注文いたします

I hereby swear to strictly follow the following upon ordering and eating the dishes served at your facility.
I acknowledge that everything I order and eat along with the menu description shall be taken at my own judgment and risk, and you shall not be liable for any physical anomaly which may occur to me.
I also acknowledge that you shall not be liable for any physical anomaly which may occur to my attendant to whom I recommend to eat.
I am over 18 years of age in perfect health condition and I have no injury or sickness but loves to eat spicy foods, so I order this menu based upon my own judgment that I am eligible to order the said food.
