料理名の通り、注文したことも食べたこともすべては私の判断で自己責任であり、身体にいかなる異常が起きても貴店には一切責任がないことを認めます

私の同伴者にすすめ、同伴者が食し、その同伴者の身体にいかなる異常が起きても貴店には一切責任がないことを認めます

私は18歳以上の健康体そのもので、けがや病気をしていない辛い物好きですので、この商品を注文する資格があると自身で判断し注文いたします

I swear herein that I order the meal at your facilities provided and I will follow the rules the following, prior to eat it.
This is my decision and my own self responsibility for any my order as stated in the name of dish, if mishaps or problem will be arisen for my body. There is no absolutely responsibility for your shop.
I also admit there is no responsibility for your shop if any of problem will be occurred to accompanied person if he or she will eat because of my recommendation .
I am 18 years old and remains healthy. I like hot food very much, of course I have no injuries and sickness at the moment.I am strongly confident to place order this food.
