残念ながら日本語の申込用紙しかありません。
2種類とも田中さんにご記入いただき、弊社に提出してもらうだけで問題ありません。
申込用紙を提出いただいた後、1週間後にはご利用いただける状態になります。

There is a need for you to fill up and submit the application form in the attachment file.

There will be no issues if Tanaka san can fill in the 2 types and submit this to us.

You must fill out the attached file, and submit it.
We regret to say that we have an application form only in Japanese.
We do not have a problem if Tanaka fills out both forms and submit them to us.

Regarding your application to use our campany's meeting space.
Unfortunately we have only the application fomat written in Japanese.
You can ask Tanaka-san for filling two fomat. it is no proalem.
After submit those to us, you can use the meeting space in a week.
