仕上げ工程での整列並べ時に水検マークが無いものに気づいたが、異常打上げせず、水検マークだけを付けて流した。

未加工防止のFP装置(加工後にワークロック解除する)はあったが、他工場と違う方法での管理させていたが、誰も気づかなかった。

コネクタ逆向きで組付けた場合、治具受けには精度上半嵌合出来てしまう構造であったため、誤ってOK判定してしまった。

前日の不良品を処置し、気付いた点は情報として流す。
設変・工変情報を(都度)流す。
品質問題等で作業者へ指示を出す時は、「前工程から不良が流れてきたら直ぐ連絡してくれ!」ではなく、「前工程からこの部品のここのキズ不良が流れてきたら直ぐラインを止め、リーダーに連絡してくれ!」と具体的に表現し、指示する。
服装の乱れも確認し、気付いたらその場で注意する。

従来から工程内不良として検出されていたが、自動化設備でのFPを過信し、真因究明・対策が遅れてしまった。

設備チョコ停後の復帰時に、段取りマンがコネクタを表裏逆セットし、プログラム上1台目のみFP機能が働かず流出。

原因不明のまま、目視チェック追加のみに頼り、勘違いで流出した。

FP装置等の設備を過信して、真因究明を怠っていることはありませんか?
不良対策として、安易に目視チェックのみに頼ることはないですか?

慢性不良として片付けず、原因究明・対策を愚直に行うことが大切です

Translation

過去にカプラ芯金による変形不具合があり、社内の水検カプラの長さ見直しを行ったが、別工場での対策抜け(旧型)があった。

他工場、仕入先で起きた横展は確実に現地、現物で確認していますか?
過去トラは、末端作業者まできちっと伝え、指導教育を行っていますか?

応援作業の分担は作業者任せにせず、監督者が明確に指示してください

ろう付後の形状修正を治具で行っていたが、手直し要領がなかったため、治具が正しく(クランプせず)使われずに接続部が変形した。

通常なら毎日手直しするところ、減産により1週間程度溜めてから修正したことで、時効硬化で硬くなり、過大な修正時の力で変形。

時効硬化…アルミ合金で焼入れ等の熱処理後、放置すると 時間の経過と共に硬化する現象.

車両に組付け様とした所、コンプレッサにコネクタが接続出来ない不具合が発生。

The the shape correction after the brazing was done with a jig. However, there was no work procedure. Therefore the jig was not used properly (it did not clamp), and the joint area got deformed.

Normally the correction is done every day. However, it was done one week later by storing samples due to production cut. So the samples got hardened over time, and got deformed by excessive force at the correction.

Age hardening: The phenomenon of aluminum alloy; If it is left for a while after the heat treatment such as quenching, it will be hardened over time.

Failure occurred as follows: When a worker tried to assemble the component to a vehicle, he could not bond the connector to the compressor.

相手嵌合チェックでは、ワークの持ち方によっては不良検出(コネクタ浮き)しづらいが、持ち方まで決まっていなかった。

いつも不良がないということで漫然と出来栄えチェックしている工程はないですか?
出来栄えチェックですり抜け、後工程で見つかった不具合は無いですか?

通常は一人で行う作業であったが、負荷が高くなり応援作業者が入ったが、各々の作業分担を曖昧にしたまま共同作業をした。

二人作業になったことにより、出来高が増え、次工程の検査に一時的にワークが溜まり、慌てて検査したことにより見逃した
