通常は一人で行う作業であったが、負荷が高くなり応援作業者が入ったが、各々の作業分担を曖昧にしたまま共同作業をした。
二人作業になったことにより、出来高が増え、次工程の検査に一時的にワークが溜まり、慌てて検査したことにより見逃した

Is there any failure escaped from the performance check and found in the subsequent process?
The work was usually done by a single worker, but there were so much labor that a supporting worker joined. However, the collaborative work was done with the division of labor being vague.
Since the work was done by two workers, the output quantity was increased. And the products piled up in the subsequent process temporarily.
The worker inspected them in a hurry and the failure was overlooked.

Depending on how work is assigned, detecting defects (loose connectors) during evaluation of coupled connectors fit can be a challenge; work assignments were not clear.
Is there a step where defects are rare and finishing check is done casually?
Are there defects that were not detected at the finishing check but discovered in a later step?
The work was usually done by one person but since the work load increased, additional helper joined, and they worked together with ambiguous work responsibilities.
As a result of working as a pair, production increased, work pooled at the check point at the next step temporarily and defects were undetected due to rushed checking.
