The following failure occurred again: Connector bonding area was deformed like an ellipse.
In the past there was a deformation failure due to the coupler core metal, and the length of water inspection coupler in the company was reviewed. However, there was one without action (old type) in another plant.
Is the cross development that occurred in another plant and the supplier checked at the site using the sample without fail?
Is the lesson learned from the past trouble told to the end workers in a proper way, and are the workers trained properly?
All the workers should share the cross development information, and the sample should be checked at the site to see if there is similar failure.

In the past, we've seen the fail change in shape because of the cored bar of couplers and reviewed the length of sampling water couplers, founding that the other factory had no action and used the older one.
Do you confirm on-site and on-the-spot the common incidents which have happened in other factories or our suppliers,?
Do you tell the whole workers about the past troubles and educate them?
The cross-available information should be shared among all workers and you should affirm that there is not the same problem with checking the actual site and kind.
