①設備や工程の変更時、②製品切り替え時(段取り替え)、③人の入れ替え時、中でも多いのは、④手直し作業時に発生する頻度が高くなっております。
また特異な例として連休明けや久しぶり生産品の初号機も非定常作業です。

整列並べのスペースが少なく、ワーク同士を重ねて置く様なやり方であった為、未加工に気づかず流出した。
納入先にてホース曲げ部が完全に曲げられていないものが発見された。
1点締めコネクタが逆向きに組付いているという不具合が発生した。

設変・工変情報を(都度)流す。
品質問題等で作業者へ指示を出す時は、「前工程から不良が流れてきたら直ぐ連絡してくれ!」ではなく、「前工程からこの部品のここのキズ不良が流れてきたら直ぐラインを止め、リーダーに連絡してくれ!」と具体的に表現し、指示する。
服装の乱れも確認し、気付いたらその場で注意する。
