保育業務委託契約書

●●（以下「保護者」）と、株式会社ABCが運営する「XXXX」（以下「当幼児園」）は、当幼児園を利用する園児●●（以下「園児」）の保育について、以下の通り委託契約（以下「本契約」）を締結しました。

第１条（契約の目的）
園児の健やかな成長のため保護者と当幼児園は互いに協力し合いながら、当幼児園は、児童福祉法、保育所保育指針、認可外保育施設指導監督基準の趣旨に従い、園児が安心して生活できる保育を提供し、保護者は、当幼児園に対して園児の保育に対する料金を支払います。

Childcare service consignment contract

●●（Hereinafter referred to as "guardian") and "XXXX" owned by ABC Co. Ltd. （エ）園児に、心臓疾患、てんかん若しくはアレルギー体質等、当幼児園が園児に対し保育サービスを提供する過程で特別の配慮を要する持病、疾患、傷病若しくは特異体質等があることが判明した場合
（オ）園児に、持病、疾患、傷病若しくは特異体質等、保育サービスを提供する過程で特別の配慮を要する事情があることにより、当幼児園として、保育のサービスの提供を継続することに特有の困難が伴うと判断した場合

園児と保護者もしくは保護者の関係者は、送迎時間の5分前までには必ずバス停で待機し、送迎時間になってもいない場合、園児の乗降を行わず、出発する。
その場合、保護者もしくは保護者の関係者は、当幼児園まで送迎を行うものとし、降園時の場合、延長保育利用料を請求する。

b. 園バス送迎以外
登園前に、検温とトイレを必ず済ませる。
登園時間は8時30分～9時00分とする。降園時間は16時30分～17時00分とする。
8時30分より前の登園及び17時00分より後の降園は、延長保育利用料を請求する。

Inspect temperature and toilet must be done before coming to kindergarten.
The time to coming is between 8:30 and 9:00. The time to leaving is between 16:30 and 17:00.
We shall request extended child care fee coming before 8:30 or leaving after 17:00.

d.その他費用の支払方法・支払時期については、別途定める。
e.保護者は、園児が月の途中で入園又は退園した場合でも、入園又は退園をした月についての、１カ月分の月額保育料及び施設利用料を支払うものとする。

13.園児送迎
保護者もしくは保護者の関係者は、園児の送迎の際に当幼児園が発行する「保護者証」を必ず携帯し、求めがあれば必ず当幼児園のスタッフに提示を行うこと。降園時（送迎バスの降車時含む）に提示が無い場合、原則当幼児園スタッフは園児を引き渡さない。

6.欠席
園児が欠席するときは、保護者は、事由を当幼児園に連絡しなければならない。特に、別表「登園禁止の感染症」にかかったときは、学校保健安全法施行規則により出席停止となり、直ちにその症状を連絡しなければならず、登園再開のためには別に登園届を提出しなければならない。また、別表にない病気においても、園から登園届の提出を求める場合がある。
集団生活をする中で、園内での感染防止のためにも、登園届の提出が無い場合には、再登園は認められない。

6.Absent
Guardian must notify the reason to this kindergarten when a child is absent. Especially, when it comes to the attached table "infectious disease forbidden to come to kindergarten", attendance shall be suspended by the School Health and Safety Law enforcement regulation, and immediately notify the symptoms, and it is necessary to submit the registration for coming to kindergarten separately in order to reentry. There are occasions to ask for submission of the registration for coming to kindergarten from the garden even in the diseases which are not listed on the table.
Without submission of a registration for coming to kindergarten, it shall be not allowed to reentry for collective living and also for preventing infection in the kindergarten.

再登園の条件は、他園児と同様に集団生活を送れることであり、「体調万全でないため、外遊び無し希望」等の要望とともに登園させることはできない。

7.遅刻
遅刻をする場合は、あらかじめ、その旨と登園予定時刻を当幼児園に伝えなければならず、外遊びや昼寝、その他活動の時間帯を避けての登園をお願いすることがある。登園予定時間の変更は、都度必ず連絡をすること。
集団行動を行い、生活リズムを築いていくためにも、保護者は、遅刻が無いよう努めること。

7.Being late

8.早退
早退をする場合は、あらかじめ、その旨と降園予定時刻を当幼児園に伝えなければならず、外遊びや昼寝、その他活動の時間帯を避けての降園をお願いすることがある。降園予定時間の変更は、都度必ず連絡をすること。
また、園児が体調を崩した場合には、園内での感染防止や園児自身の健康を守るためにも、直ちに当幼児園まで保護者もしくは代理の人にお迎えいただく。

9.入園
当幼児園に入園するには、別に指定する日までに入園料の支払い手続きをしなければならない。

a.園バス送迎
バス乗車前に、検温とトイレを必ず済ませる。

11.修了
理事長は、園児が所定の課程を修了したと認めるときは、卒園証書を授与する。

12.保育料等
当幼児園の保育料等は、別紙「保育業務委託契約書」の通りである。
a.月額保育料、施設利用料、昼食費（おやつ込み）及びバス送迎費は当月20日に翌月分を支払い。

11.Completion

12.Child care fee etc
Child care fee etc shall be followed as attached "Childcare Service Outsourcing Contract".
a.Pay the following month on the 20th of the month, the monthly childcare fee, facility usage fee, lunch fee (snack included) and bus transfer fee.

b.延長保育利用料は、翌月の20日に当月分を支払う。但し、退園月や卒園月については、この限りでない。
c.上記の保育料等の支払いは、原則として保護者指定の金融機関口座から自動引落とする。20日が土日祝日等金融機関の休業日の場合、翌営業日に引き落としを行う。ただし、口座振替手続き完了前や残高不足等の理由で引き落としがされない場合、別に指定する銀行口座へ所定の金額を振り込まなければならない。振込名義人は「園児の氏名」とし、園児の氏名と異なる場合は、当幼児園へ連絡を行うものとする。

10.退園
園児が退園する場合は、退園希望の前々月の末日までに、保護者から理事長に退園届を提出するものとする。なお、以下のいずれかに該当する場合、書面にて保護者に通知することにより、当幼児園は事前の催告又は通知をすることなく、園児を退園とすることができる。
a.保護者が保育料等を所定の期日を過ぎても支払わない場合。
b.園児の保護者等の関係者が、園規則など園の運営ルールを守らない場合。

10.Withdrawal
When child withdraw from this kindergarten, it shall be submitted an withdrawal notification from the guardian to the president by the last day of the month before the desire to leave. 1.In the case that parents do not pay child care fees and others even after the prescribed deadline.
2.In the case that stakeholders of guardians of a child do not observe the kindergarten management rules such as the garden rules.
