This is not the document what I need.
My understanding is that this is another document that you created based on C88.
I would like to have a copy of original C88 like the attached document.
Please also find the link for HMRC below.
When you import goods to UK, do you submit this kind of document?

Thanks for your message. This is not the document I want.
I understand that this is another material your company made based on C88.
I'd like to have a copy of original C88 as in the attached document.

Doesn't your company submit document like this when importing parcel to UK?
