4.[NAME]フィールドは、厳密には姓・名に別れており、[KANA]も同様にわかれています。また日本では使用することがありませんが、欧米人用に[Middle Name]フィールドとその[KANA]フィールドもあります。
現在は[NAME],[KANA]それぞれ1つづつのフィールドしか用意されていませんので、最低限「姓・名」フィールドに分けることが望ましいと思います。
