I am sorry to hear that your shipment did not arrive in its expected state. I do see that you have purchased insurance and we will need to file a claim with the carrier and our Claims Department. Please complete and sign the attached document and email this information to us at [e-mail] along with the following supporting information:

1. Copy of the merchant invoice or receipt(s)
2. Completed and signed Claim Form (attached to email)

If an item is damaged in the shipment you will need to provide this additional information:
