○職歴の欄は「どこに所属して」「何をした」ということを、ごく簡単に記載してください。
○最下段の日付の欄は空欄にしておいてください。
押印をして、最終日までにいただきたいです。
私がいなかったら、他のスタッフに渡してください。

I have to submit the instructor's resume of qualification before submitting my completion report to the Institute A.
Attached please find the format and sample of that, so please fill it by manuscript or by typing.
- As for the section of your career, please fill it simply such as "I belonged to xxx" and/or "I did xxx".
- As for the last section of the date, please leave it blank.
Please stamp your seal and send me before the deadline.
If I am not present, please pass it to other staff.
I would appreciate your kind cooperation, thank you.
