「OSHMS」は、Occupational Safety and Health Management Systemの頭文字です。
日本では、厚生労働省から「労働安全衛生マネジメントシステムに関する指針（OSHMS指針）」
（平成18年改正。）が示されています。
氷山（大きな失敗）は海面下に巨塊（小さな失敗）が隠れている。
さらにその裏には、300件の社員が「しまった」と思っているが外部の苦情がないため見逃しているケース、つまり認識された潜在的失敗が必ず存在するといえる。
