メールアドレス等の個人情報の収集は一切行わず、履歴PDFの送信以外の使用は致しません。尚、個人情報を除いた履歴は予防医学向上のための研究として使用することをご承知ください。

NPO法人 地域診療連携協議会のコンソーシアムに参加しています。
2012年4月現在、対応言語は日本語、ひらがな、英語、中国語（簡体）、中国語（繁体）、韓国語、インドネシア語、フランス語、スペイン語、ポルトガル語の10言語となっております。
対応言語は順次追加する予定です。

※ The health life passport is developed by the collaborative investigation of Eye S gate Co., Ltd. and Gunma University medical department medical care information bureau.
It participated in the consortium of the NPO corporation area medical treatment cooperation meeting.
On April, 2012, the correspondence language becomes to 10 kinds of languages such as Japanese, hiragana letters, English, simplified Chinese, Traditional Chinese, Korean, Indonesian, French, Spanish and so on.
The correspondence language is going to be added sequentially.

Health Life Passport was developed in cooperation with I.S.Gate company and department of medical information in Gunma Medical School. It has participated in a consortiuman of incorporated non-profit organization, community clinical association.
In fact, in April 2012, it has served with 10 languages such as Japanese, Hiragana, English, simplified Chinese, traditional Chinese, Korean, Indonesian, French, Spanish, Portuguese. There are plans to make more languages available in the future.
