商品XXXの大ヘッドが接触不良で動きません。替えのヘッドを送ってください。
また先日送っていただいた商品XXXの中ヘッドですがコードがなく使えません。
コードが切れていたのでヘッドだけでなくヘッドのコードも必要です。

The big-sized head of the product xxx does not move because of the poor contact. Please send me another one.
Also the middle-sized head you sent me the other day does not work because the code was not included.
The code was cut off so I need not only another head but also the code of the head.
So what I need are:

2 The code of the middle - sized head

Since the big head for the item XXX has not been working due to lose contact, please send me the replacement asap.
And, the medium size head for the item XXX I have received recently, it cannot be used since the cord was not included with.
Because the cord has been broken apart, the necessary replacement has to be not only the head but also the cord for head.
The followings are the necessary replacement I would like you to ship to me.
1. Big Head: 1 pc.
2. Cord for medium size head: 1 pc.
Please ship those to the address below.

The large head of the item XXX doesn't work because of the connection failure. Send an alternative one, please. And I can't use the middle head of the item XXX because its cable has broken. I need not only the middle head itself but also an unbroken cable. Totally I need;
