●A 47-year-old man was admitted to this hospital because
of abdominal pain.
The patient was in his usual state of health until 5 weeks before admission, when
diffuse abdominal pain developed, most severe in the lower right quadrant and radiating
to the right side and back; it decreased slightly with leaning forward, increased
with other movements, including walking, and prevented him from sleeping.
He went to an urgent care clinic at a health center affiliated with this hospital.


Computed tomographic (CT) scans of the abdomen and pelvis 4.5 years before admission,
obtained after the oral and intravenous administration of contrast material,
were interpreted as normal.

４７歳の男性は腹痛によりこの病院に入院した。
この患者の入院５週間前までの健康状態は良好であったが、右下腹部４分の１区と右側と背中にびまん性の腹痛が起こった時、前のめりになることで少し楽になったが、歩いたり他の動きをすることで痛みがひどくなり眠りを妨げるまでになった。患者はこの病院付属の医療センターの緊急医療センターを訪れた。

入院の４～５年前に腹部と骨盤を撮ったコンピューター断続撮影（ＣＴ）では、撮影の前に造影剤を経口投与と静脈投与されたものが正常であると診断されていました。

入院する前の5週間、患者は普通の健康状態だったが、入院時、腹痛が広がり、右四半部下の方が特にひどく、右側と背中に向かって痛みが放射状に広がった。前かがみになると痛みは落ち着き、歩くなどの別の動きをするとひどくなり、眠ることもできなかった。
患者はこの病院の提携の健康センターの応急診療所に運ばれた。

経口と静脈注射による造影剤の投与後に入院の4,5年前の腹部と骨盤のコンピュータ断層撮影(CT)スキャンが得られたが、異常はなかった。

この患者は、承認がでる5週間前までは、普段の健康状況を認め、
腹部の痛みが広範囲に広がった際、最も痛いのは、右下腹四半部と、放射状に広がる右横と後腹部、その痛みは、前かがみになる際、弱まり、歩くことなどを含むその他の行動の際には、痛みが増していました。それが原因でよく眠れず、この病院が提携しているヘルスセンター内の、救急ケアセンターへ来院されました。

腹部ＣＴスキャンでの結果、承認前の過去4,5年、経口投与と、静脈内投与、造影剤は、正常だと解釈されていた結果を入手いています。
