I was able to get in touch with MYusa.com and found out what happened. I found out the freight forwarder uses the Suite # to determine who the package belongs to. Since we had shipped similar products before to this exact same address, the factory did not enter the correct suite number and just sent it to the address they already had in their system.
package was addressed to ....

The factory should have placed this on the label....

MYusa.comと連絡を取ることが出来、状況がわかりました。運送会社が荷物の持主を確定するために組番号を使用していることがわかりました。過去にちょうどこれと同じ住所に類似の製品を配送しましたので、工場は正しい組番号を入力せずに、すでにシステムの中に入っていた住所に送っておりました。
荷物はXXXXXXXX宛に配送されました。
工場はラベルにこれを貼るべきでした…
