クレジットカードの明細を FAX して欲しいとのことですが、
現在は Web で明細を確認するようになっており、ご指定いただいた
「クレジットカード番号の下４桁」「住所」が明細では確認できません。
カードそのものは手元にあるので、それを写真かなにかで送るというのは
難しいでしょうか？
また、住所に関してはクレジットカードが私のところに郵送されて来たと
きのものでは確認になりませんでしょうか？
他に方法があればご指示ください。

I'd like to ask you to fax us your credit card details, currently the details are confirmed through the web. We are unable to confirm the last four digits of your credit card and the address you entered.
Since you have the card with you, would it be possible to send us an image of it or something like that?
Regarding the address, can you confirm that the card is the one that was shipped to me?
