アフタースクールのダンスレッスンの見学会を行います。

この3ヶ月の練習の成果を披露します。
まだ3ヶ月という経験の浅い子供たちではありますので、showという形で
発表をするというイメージよりは、普段のレッスンの様子とこの3ヶ月で
学んだ振り付けをお見せするような、カジュアルな内容となります。

Explanation of details for entrance, written form and others

Since we try on uniforms and other items (size check) on the day, please come with your children.
Please refrain from eating and drinking other than water and tea.

We will organize a tour of after-school dance lessons.
The dance lesson has just started in January this year, we will show off the achievements of this 3 month exercise.
Because these children have experienced just for 3 months, it will be casual content to show you the normal lesson and choreography they learned in this 3 months rather than from the image of presenting in the form of show.

保育業務委託契約書

●●（以下「保護者」）と、株式会社ABCが運営する「XXXX」（以下「当幼児園」）は、当幼児園を利用する園児●●（以下「園児」）の保育について、以下の通り委託契約（以下「本契約」）を締結しました。

第１条（契約の目的）
園児の健やかな成長のため保護者と当幼児園は互いに協力し合いながら、当幼児園は、児童福祉法、保育所保育指針、認可外保育施設指導監督基準の趣旨に従い、園児が安心して生活できる保育を提供し、保護者は、当幼児園に対して園児の保育に対する料金を支払います。

Childcare service consignment contract

●●（Hereinafter referred to as "guardian") and "XXXX" owned by ABC Co. Ltd. (hereinafter referred to as "the kindergarten") are contracted entrusted a consignment contract (hereinafter referred to as the "Agreement") as follows regarding childcare of the child ●● (hereinafter referred to as "child") using this kindergarten.

Article 1 (The purpose of the contract)
In accordance with the purpose of the Child Welfare Law, Nursery School Education Guidelines, Nursery School Childcare Guidelines, Non-Child Care Facilities Guidance and Supervision Standards, the kindergarten shall provide nurseries that allow children to live with peace of mind, guardian shall pay a fee for nursery to the kindergarten through cooperating each other by guardians and the kindergarten for the healthy growth of the child.

（エ）園児に、心臓疾患、てんかん若しくはアレルギー体質等、当幼児園が園児に対し保育サービスを提供する過程で特別の配慮を要する持病、疾患、傷病若しくは特異体質等があることが判明した場合
（オ）園児に、持病、疾患、傷病若しくは特異体質等、保育サービスを提供する過程で特別の配慮を要する事情があることにより、当幼児園として、保育のサービスの提供を継続することに特有の困難が伴うと判断した場合

(d) In case, there is a chronic disease, disease, injury or idiosyncrasy and others, such as heart disease, epilepsy or allergic constitution and others, that require special needs in the process of providing nursery service to the child by the kindergarten.
(e) In case, it is determined that is difficult for the kindergarten to continue to provide nursing services for child due to circumstances requiring special needs in the process of providing nursery services to children, such as chronic illness, illness, injury or idiosyncrasy.

園児と保護者もしくは保護者の関係者は、送迎時間の5分前までには必ずバス停で待機し、送迎時間になってもいない場合、園児の乗降を行わず、出発する。
その場合、保護者もしくは保護者の関係者は、当幼児園まで送迎を行うものとし、降園時の場合、延長保育利用料を請求する。

b. 園バス送迎以外
登園前に、検温とトイレを必ず済ませる。
登園時間は8時30分～9時00分とする。降園時間は16時30分～17時00分とする。
8時30分より前の登園及び17時00分より後の降園は、延長保育利用料を請求する。

Children and their guardians or stakeholders must wait at the bus stop at least 5 minutes before the pick-up time for kindergarten. We shall leave without getting on and off of a child if they do not show up at the pick-up time to Kindergarten.
In that case, guardians or stakeholders of children shall pick children up and take them to this kindergarten, and charge extra child care fee when it is at the time of leaving the kindergarten.

b.other than kindergarten bus transportation
Inspect temperature and toilet must be done before coming to kindergarten.
The time to coming is between 8:30 and 9:00. The time to leaving is between 16:30 and 17:00.
We shall request extended child care fee coming before 8:30 or leaving after 17:00.

振込手数料は保護者の負担とする。
d.その他費用の支払方法・支払時期については、別途定める。
e.保護者は、園児が月の途中で入園又は退園した場合でも、入園又は退園をした月についての、１カ月分の月額保育料及び施設利用料を支払うものとする。

13.園児送迎
保護者もしくは保護者の関係者は、園児の送迎の際に当幼児園が発行する「保護者証」を必ず携帯し、求めがあれば必ず当幼児園のスタッフに提示を行うこと。降園時（送迎バスの降車時含む）に提示が無い場合、原則当幼児園スタッフは園児を引き渡さない。

Payment fee shall be borne by parents.

e.Guardians shall pay monthly childcare fee and facility usage fee for one month of the month which child entered or withdrew even if the child enters or leaves in the middle of the month.

13.Pick up children
Guardians or stakeholders of guardians shall be sure to carry a "guardian card" issued by this kindergarten at the time of transferring the child, and show to kindergarten staff if they request. In the absence of presentation at the time of leaving from kindergarten (including getting off the shuttle bus) In principle, this kindergarten staff shall not hand over the child.

6.欠席
園児が欠席するときは、保護者は、事由を当幼児園に連絡しなければならない。特に、別表「登園禁止の感染症」にかかったときは、学校保健安全法施行規則により出席停止となり、直ちにその症状を連絡しなければならず、登園再開のためには別に登園届を提出しなければならない。また、別表にない病気においても、園から登園届の提出を求める場合がある。
集団生活をする中で、園内での感染防止のためにも、登園届の提出が無い場合には、再登園は認められない。

6.Absent
Guardian must notify the reason to this kindergarten when a child is absent. Especially, when it comes to the attached table "infectious disease forbidden to come to kindergarten", attendance shall be suspended by the School Health and Safety Law enforcement regulation, and immediately notify the symptoms, and it is necessary to submit the registration for coming to kindergarten separately in order to reentry. There are occasions to ask for submission of the registration for coming to kindergarten from the garden even in the diseases which are not listed on the table.
Without submission of a registration for coming to kindergarten, it shall be not allowed to reentry for collective living and also for preventing infection in the kindergarten.

再登園の条件は、他園児と同様に集団生活を送れることであり、「体調万全でないため、外遊び無し希望」等の要望とともに登園させることはできない。

7.遅刻
遅刻をする場合は、あらかじめ、その旨と登園予定時刻を当幼児園に伝えなければならず、外遊びや昼寝、その他活動の時間帯を避けての登園をお願いすることがある。登園予定時間の変更は、都度必ず連絡をすること。
集団行動を行い、生活リズムを築いていくためにも、保護者は、遅刻が無いよう努めること。

7.Being late
In case of being late, it must be notified that and the scheduled time for entry to this kindergarten beforehand, we shall sometimes asked to avoid playing outside, taking a nap, other time zones of activity. The change of scheduled time for entry shall be notified every time.
Guardians should try not to be late for collective living and build a life rhythm.

8.早退
早退をする場合は、あらかじめ、その旨と降園予定時刻を当幼児園に伝えなければならず、外遊びや昼寝、その他活動の時間帯を避けての降園をお願いすることがある。降園予定時間の変更は、都度必ず連絡をすること。
また、園児が体調を崩した場合には、園内での感染防止や園児自身の健康を守るためにも、直ちに当幼児園まで保護者もしくは代理の人にお迎えいただく。

9.入園
当幼児園に入園するには、別に指定する日までに入園料の支払い手続きをしなければならない。

9.Admission
The entrance fee shall be paid by the date which specified separately to enroll in this kindergarten.

代理人で、保護者証を提示できない場合は、保護者が代理人の氏名を当幼児園へ伝え、代理人は、送迎時に身分証明書を提示しなければならない。
園児の体調や朝の体温を保護者は当幼児園の所定の方法にて報告し、原則、体温37.5度以上の場合、当幼児園は受け入れない。また、熱が無い場合でも、園児の健康状態によっては、受け入れられない場合がある。

a.園バス送迎
バス乗車前に、検温とトイレを必ず済ませる。

If the alternative person who can not present the guardian card, guardian shall notify the name of him/her to this kindergarten and he/she must show their ID at the pick up.
Guardian shall report the condition body temperature in the morning of the child by the prescribed means to this kindergarten and principally, when child body temperature is 37.5 degrees or more, We shall not accept to enter this kindergarten. Even if child do not have the fever, it may not be accepted depending on the health condition of the child.

a.Kindergarten bus transfer
Child must be done the inspect temperature and toilet before riding the bus.

c.園児の保護者等の関係者が、園の運営を著しく阻害する場合。
d.その他、当幼児園長と保護者の間で協議し、当幼児園の利用を継続することが園児の健やかな成長を妨げると判断した場合。

11.修了
理事長は、園児が所定の課程を修了したと認めるときは、卒園証書を授与する。

12.保育料等
当幼児園の保育料等は、別紙「保育業務委託契約書」の通りである。
a.月額保育料、施設利用料、昼食費（おやつ込み）及びバス送迎費は当月20日に翌月分を支払い。

c.When it is significantly inhibited the management of the kindergarten by child guardians or their stakeholders
d.When it is the others, judged that continuing the use of this kindergarten shall interfere with the healthy growth of the children through discussion between director of this kindergarten and guardians/

11.Completion
The president shall award the graduation certificate when he/she admit the child has completed the prescribed course.

12.Child care fee etc
Child care fee etc shall be followed as attached "Childcare Service Outsourcing Contract".
a.Pay the following month on the 20th of the month, the monthly childcare fee, facility usage fee, lunch fee (snack included) and bus transfer fee.

b.延長保育利用料は、翌月の20日に当月分を支払う。但し、退園月や卒園月については、この限りでない。
c.上記の保育料等の支払いは、原則として保護者指定の金融機関口座から自動引落とする。20日が土日祝日等金融機関の休業日の場合、翌営業日に引き落としを行う。ただし、口座振替手続き完了前や残高不足等の理由で引き落としがされない場合、別に指定する銀行口座へ所定の金額を振り込まなければならない。振込名義人は「園児の氏名」とし、園児の氏名と異なる場合は、当幼児園へ連絡を行うものとする。

b.Extended childcare fee shall be paid the current month on the 20th day of the following month. However, it does not apply to the month of withdrawal and the month of graduation.
c.Payment of the above childcare fee and others shall be automatically withdraw from financial institution account designated by guardian in principle. However, it must be transferred a predetermined amount to the separately designated bank account when it is not debit for reasons such as before bank transfer procedure completion or balance shortage. The transfer holder shall be the "name of the child" and it shall be notified the kindergarten if it is different from the name of the child.

これが、指定する日までに行われない時、入園許可を取り消すことがある。

10.退園
園児が退園する場合は、退園希望の前々月の末日までに、保護者から理事長に退園届を提出するものとする。なお、以下のいずれかに該当する場合、書面にて保護者に通知することにより、当幼児園は事前の催告又は通知をすることなく、園児を退園とすることができる。
a.保護者が保育料等を所定の期日を過ぎても支払わない場合。
b.園児の保護者等の関係者が、園規則など園の運営ルールを守らない場合。

10.Withdrawal
When child withdraw from this kindergarten, it shall be submitted an withdrawal notification from the guardian to the president by the last day of the month before the desire to leave. If any of the following applies, this kindergarten shall be able to withdraw the child without giving advance notification by notifying the guardians in writing.
1.In the case that parents do not pay child care fees and others even after the prescribed deadline.
2.In the case that stakeholders of guardians of a child do not observe the kindergarten management rules such as the garden rules.
