クレジットカードの明細を FAX して欲しいとのことですが、
現在は Web で明細を確認するようになっており、ご指定いただいた
「クレジットカード番号の下４桁」「住所」が明細では確認できません。
カードそのものは手元にあるので、それを写真かなにかで送るというのは
難しいでしょうか？
また、住所に関してはクレジットカードが私のところに郵送されて来たと
きのものでは確認になりませんでしょうか？

他に方法があればご指示ください。

In response to your request for sending the credit card statement via FAX, but it is now offered over the website to check and with "the last 4 digits of your credit card" and "the address" you have provided, the details cannot be confimed.
The credit card itself is with me, so would it be okay to send the image via e-mail or something?
Also, concerning the address, would the copy of the address where the credit card was sent to me stand as a proof?

Kindly advise if anything else.
