第2条 （業務）

第3条 （報告）
　受託者が患者と契約を締結したときには、直ちに契約書を委託者に送付し、その内容を報告するものとする。
第4条 （手数料）
　手数料は、治療費の　　％とする。
　毎月末日においての患者数と費用を計算のうえ、翌月２０日までに、計算書を送付するとともに、受託者の指定する口座に送金して支払う。
