第2条 （業務）
　受託者は、委託者の名を持って硅谷医院の宣伝斡旋を行い、委託者の指示する内容に従って患者に説明し、病院の出す診断見積価格をもって、患者との契約を締結する。
第3条 （報告）
　受託者が患者と契約を締結したときには、直ちに契約書を委託者に送付し、その内容を報告するものとする。
第4条 （手数料）
　手数料は、治療費の　　％とする。
　毎月末日においての患者数と費用を計算のうえ、翌月２０日までに、計算書を送付するとともに、受託者の指定する口座に送金して支払う。

Article 2 (duties)
The trustee performs advertising mediation of Guigu Hospital with the name of the consignor and explains it to a patient according to the contents which the consignor orders and, with the diagnosis estimated value that the hospital starts, makes a contract with the patient.
Article 3 (report)
When a trustee made a contract with a patient, I send a contract to a consignor promptly and shall report the contents.
Article 4 (fee)
The fee assumes it % of treatment costs.
I send a check by 20th in the next month and I remit to the account which the trustee appoints and pay the number of patients and the expense in the last day after a calculation every month.
