私は、「Annual License Options」での申請を考えております。どのようにすればよろしいでしょうか？

申請したい内容は、先日お送りした下記メール及び添付ファイルのとおりです。

申請に先立ち、添付ファイルの内容が、書籍「ABC（ISBNナンバー123）」に含まれていることを確認したいです。
