2.私は身体的に健康で、本アクティビティにより影響を受ける状態でないことを明言いたします。
３.私は参加する本アクティビティが、自然環境の中での体験であること、またそれに伴う危険性を認識しガイド・スタッフによる安全に関する説明を理解し指示に従います。その限りでない場合に生じた身体及び精神的損害については責任を問いません。
Application and acceptance form to attend
1. I will take part in this activity of my own accord - without any compulsions.
2. I will clearly say that I am phisically healthy and will not be affected by this activity.
3. I know that this activity is perfomed in the natural environment and recognize that there might happen anything dangerous. Therefore I will understand and follow the safety manual conducted by the guide staff. If I have an accident under other circumstances, I will not hold you to account.
