私、＿＿＿＿＿＿＿（認可バイヤー）は、エンターテインメント・アースInc.の一部門であるEEディストリビューション（マーチャント番号160014002572）に対し、必要に応じて複数の取引を使って＿＿＿＿＿＿＿（貴社名）に対して発生する課金について以下のクレジットカード／デビットカードで支払い、送料、その他の手数料を処理することを、取り消し不能に許可します。
This form is
necessary even if other payment arrangements have been made in case of processing or shipping fees. Only credit/debit cards authorized
on this form can be used to place orders. I understand that by filling out this document I have authorized Entertainment Earth, Inc. to process
charges on these credit/debit cards until the card has expired or I have received acknowledgement from Entertainment Earth of my written
notice expressing my desire to cease and desist use of the credit/debit card.
