Sometimes when they assemble the parts before the painting happens I'm sure small pieces fall loose inside and this in common as long as the outer finish is ok lladro quality control will pass as first quality.
Sometime you may encounter a piece that has the tulip flower removed this would indicate lladro has deemed the piece as grade 2 or less than perfect.
This small rattle you mention is often found and was not enough for lladro to worry about and that limited peicehas the full flower so I think there is no problem.
