に封入されている連動シリアルコードをそれぞれ、1つずつ（合計2つ）ご入力ください。
各シリアルコードをご入力いただくと、応募完了となります。（1口分）
複数口応募可能ですが、その際は、LIVE映像商品と5thアルバムのシリアルコードを2点必ずご入力ください。
