雇入れ時健康診断のご案内
雇入れ時検診の受診機関について
ご入社時には健康診断の結果をご提出いただくことになっており、弊社提携の病院で無料で受診をいただいております。健康診断は予約制となりますので、必ず事前に電話にて予約をお願いいたします。
予約の際は「Xの雇入れ時健康診断」であるとお伝えください
受診項目は以下になります
1既往歴及び業務歴の調査
2自覚症状及び他覚症状の有無の検査
3身長，体重，腹囲、視力及び聴力の検査
4胸部エックス線検査
5血圧の測定
6貧血検査（赤血球数，ヘモグロビン）
Notice of Physical Examination upon Employment
Where to have a medical examination;
Every prospective employee is required to submit a result of medical examination. They can have the examination at the designated hospital tied up with our company for free of charge.
健康診断は予約制となりますので、必ず事前に電話にて予約をお願いいたします。
As the examination is conducted on reservation basis, please make reservation using telephone in advance.
When you have the medical examination, please tell them that it is “the examination for prospective employee.”
Examination includes following items;
1. Examination on past illness and anamnesis
2. Examination of a subjective and a objective symptoms
3. Measuring of height, weight, chest, and examination on the eye sight and the hearing ability.
4. A chest X-ray examination
5. Measuring of blood pressure
6. Examination on anemia (Number of red cells and hemoglobin)
7肝機能検査、血中脂質検査、血糖検査
8尿検査（尿中の糖及び蛋白の有無）
9心電図検査（安静時心電図検査）
ご案内の健診機関では項目を指定いただく必要はございません
診断書ご提出について
上記機関で受診をいただきますと約一週間程で結果が届きますのでそちらのご提出をお願いします。またご入社日より三ヶ月以内に健康診断を受診されている場合はそちらの診断書のご提出でも結構です。ご入社までにご提出をお願いします
Yクリニックで受診された場合は、診断結果が会社に直接送られますのでご提出は不要です
7肝機能検査、血中脂質検査、血糖検査
7. Liver function test/Examination of blood lipid level/Blood sugar test.
8. Urine analysis (Urinary sugar test/Urinary protein test)
9. Electrocardiography (in resting)
Medical examinees are not required to specify the items for medical examination at designated hospital.
For the submission of the medical examination result;
If you have the medical examination at above mentioned medical facilities, you will receive the result after around one week, so please submit the received medical result.
However, we can accept the result of medical examination if you have had it within three months before your employment date. Please submit is before joining our company.
If you have the medical examination at Y clinic, you will not need to submit the result as it will be directly sent to the company.
