（注意）製造所の所在地の変更に関する留意事項
移転等により所在地の変更があった場合については、新しく認定申請を行うことになるため、変更届による届出ではありませんので留意して下さい。単なる住居表示の変更による場合は、備考欄に「住居表示の変更による」等、その旨が分かる内容を記載して下さい。
(Note) Points to be considered regarding location changes of the manufacturing site
In the case where the location has been changed due to relocation etc., please note that a new application for approval will be necessary, instead of a notification of change. If it is only a change in the residence indication, please indicate in the remarks column, such as "Due to change of residence indication".
