In the case where the location has been changed due to relocation etc., please note that a new application for approval will be necessary, instead of a notification of change. If it is only a change in the residence indication, please indicate in the remarks column, such as "Due to change of residence indication".

(Attention) Reminders on change of manufacturing location
Regarding of change of locations due to moving etc., as to apply for a new certification, please be remind that it is not a notification of change. In the case of only change of location, please put `due to the residence was changed` etc. understandable contents on remarks column.
