I further understand that if I am a current employee of the Company or any of its subsidiaries, I am required to have my shares, once converted, deposited into the brokerage account provided by the Company; a process facilitated by the company on my behalf, and no further action is required by me.

I currently am an Employee of the Company or its subsidiaries and
I would like my shares to be transferred to Morgan Stanley Smith Barney.
I wish to transfer my shares to a broker of my own choosing.

Please Note: you can only elect to convert once a day. If you proceed with this election any previous elections done today will be cancelled.

私は更に、私が現在、会社もしくはそのいずれかの子会社の従業員である場合、私に交付される株式は、それが転換され、会社が用意した私の証券取引口座に預け入れられることを了解する。なお、当該手続きは会社が私を代理して執り行うものであり、私にはいかなる行動も要求されていないものである。

私は現在会社もしくはそのいずれかの子会社の従業員であり、私の株式を Morgan Stanley Smith Barney 社に移管させたい。私は私の株式を、私の任意の証券会社に移管させたいのである。
