参加申込書および参加承諾書
１.私は本アクティビティに何人の強制も受けず自己の意思により参加いたします。
2.私は身体的に健康で、本アクティビティにより影響を受ける状態でないことを明言いたします。
３.私は参加する本アクティビティが、自然環境の中での体験であること、またそれに伴う危険性を認識しガイド・スタッフによる安全に関する説明を理解し指示に従います。その限りでない場合に生じた身体及び精神的損害については責任を問いません。
Attendance Application and Attendance Letter of Acceptance
1. I attend this activity with my own intention and without others' compulsion.
2. I declare that I am in good health and it will not be affected by this activity.
3. I am aware that this activity is held in the natural environment and of the risk involved. I understand the explanation of safety by the guide staff and I will adhere to the instructions. I will not hold this party responsible for any actions that result in physical or mental damage caused from straying from the above stated rules.
4.私は本アクティビティ催行中に撮影された写真・ビデオ等の肖像権についてはその一切を弊社に帰属します。
５.ツアーガイドは安全上の事由により本アクティビティのキャンセル及び参加者のトリップ続行を拒否する権利を有することに承諾いたします。
６.私は本アクティビティ参加日当日、飲酒・薬物等により酩酊状態にありません。７.私はツアー中、常時笑顔を忘れず、いかなる場所でも一生タバコ・ゴミの投げ捨てをいたしません。そして、私たちの地球・自然を大切に守っていくことをここに明言いたします。
4. I will allow my image right of photographs and video during this activity to be vested all in your company.
5. I will agree that the tour guide has the right to cancel this activity or to refuse to continue the trip of participants for safety reasons.
6. I will never be inebriated by drinking alcohol or taking drugs on the date of this activity.
7. I will have smiley face during the trip. I will clearly say that I will protect our green earth and never throw away litter or cigarettes anytime anywhere.
