私は昨年の１２月に３セットずつ２回、合計６セットの三脚を購入しました。
リベートを２回申請した内の１回３本分は承認され、プリベイドカードを頂きました。
しかし、同じ書類を提出している残りの３本分は、日付が入っていないとの理由で却下されたので、
今回の問い合わせに至っております。
ネットショップから購入したので、それぞれ支払い確認のメールと、トラッキングのEメールを送信させて頂きます。
日付はEメール中にありますが、トラッキングのメールを見て頂ければ、オーダーの日付が書いてあります。

I applied for the reward program twice and one time(3 sets) was approved and I received a prepaid card.
However, the rest of them,3 tripods, were not approved since the date was not shown even though I submitted the same document. So that's why I'm writing this email now.
As I purchased them from an online store, I attached the payment confirmation emails which include a tracking number of the items.
The date can be found in the email and also the date of the order can be found if you check the email contains a tracking number.
