もし、「まだ配っていない」という返事であれば、全部配ってもらうようにお願い
してもらえますか。
また、もし、「全部配りました」という返事であれば、1で余った用紙を持っていって
いただけますか？

Shall we again contact them through mail or phone?
In case if there is reply like " not yet delivered" then, could you please deliver it entirely.
And in case if there is reply like " all delivered" then can you provide one additional application form please?

2. You emailed to the distributor to distribute them all, you said, but did you get reply from them to confirm that they actually distribute them all?
Can you phone or email them again?
If they still have some left, please ask them to distribute the rest as well.
Or, if they finished distributing them all, please take the application forms that you still have as in my question 1 above.
