To Mr. (or Mrs.) Yan
I think there have been several misunderstandings between us and with this e-mail I hope to clear up those misunderstandings.
First of all, I would like to thank you for the all the help you have provided us.
Until now I have been able to take pride in the fact that I have worked for ten years on transplant support.
Our work here is not broker work but actually we primarily give support to patients, and we think of helping patients return to their home country with a successful safe surgery without any emotional anxiety as our true main mission.

Of which, first of all, what I am apprehensive about is the conversation the other day, which went from the law in Indonesia that donor offers coming from death row inmates are allowed, and then without any involvement of the doctors regarding any donors, you changed the conversation to searching for a donor.

On that note, my experience tells me that you are receiving organ donor offers from impoverished people of rural areas. In that case, do you use a lawyer along with legitimate documents?

With our mission, what we concern now is the way to find a donor. Originally, we were told that, under the Indonesian law, you can provide organs from condemned inmates legally, however it turned out that you are the one to find a donor without doctors‘ involvement.

I take it from my experiences that an organ comes from a poor person in countryside. If that is the case, does it mean that we hire a lawyer to prepare documents in order to make the procedure legal?

First of all, I am concerned about the outline of the story, which was to legally receive organ donations from the death-row inmate in Indonesia, has changed to a story finding a donor without involving a doctor but Mr. Yan.
From my experience, this means that accepting a donor from people living in an impoverished countryside. If this is the case, are you planning to prepare documents to follow the law?

Among these, at first, what I am anxious about is that, someone said accroding to the law of Indonesia, it is possible to offer a legal donor from a condemned criminal, the doctor is not related to the donor, but Jan looks for a donor.
About this, I receive organ donation from the poor person of the country in my experience, In this case, could I make documents with a lawyer to make it become legal?

I hope we can avoid the procedure, and I would like to have a legal donor offer from condemned inmates, as we discussed before.

The next thing bothering me is that I still haven’t heard from you about how much it would cost, even for a rough price. You have been told from doctors before that it would cost about 3 billion rupiahs in a black market, and it isn’t unconvincing. However, we cannot talk to patients in details without knowing how much it would cost for an organ transplant.

If that is the case, we would rather receive organ donations legally from the death-row inmate, as I mentioned before.
I am a little anxious since we haven’t got the budgetary quote yet.
It seems that, Mr. Yan has quoted approximately 300 million IDR as compensation to the doctor side, which is reasonable. But we have no idea about the donor expense, so we can’t pursue this issue with the patient at this moment.

I will not do such thing as possible, I hope for a donor offer by a condemned criminal as before.
In addition, about an expense, I fall into the point that some prospect does not appear, yet. Jan(Yan) seemed to said it is 3,000,000,00 rupiah before in a doctor side, it is not impossible to assent about this amount of money. However, because I do not know how much the donor will take at all, so I cannot explain to the patients.

Frankly speaking, the total amount we can prepare including Mr. Yan’s commission fee is $50,000 at the most. If this is impractical, I’m afraid we have to turn down this offer and take the patient back immediately.
If possible, please inform us within today regarding this issue.
I fully understand that Mr. Yan is in a financially difficult situation (according to Mr. Kobayashi) but, please allocate the commission fee to cover the daily transportation charge.

To be frank with you, the total cost we can accept is 50,000 dollars at the highest, including your commission. We can’t pay any more than that, so if you are not able to arrange it 50,000 dollars or less, it is very unfortunate, however, we have no choice but to discontinue our talk. In that case, the patient will go home immediately. If you can arrange it, please contact me by tonight.

And I am well aware that you are in a difficult situation economically, which I heard from Mr. Kobayashi, but I hope you can manage daily expenses such as transportation fares out of the commission you receive.

I express it frankly in this place, for the expenses, total of $ 50,000 including fee for Jan (Yan) is the highest limit that we can consent to. If this is impossible, I am very disappointed, I have to refuse. In that case, the patient will go back to his/her own country promptly.
I am looking forward to hearing from you by the end of today, if possible.
In addition, I heard from Kobayashi that the economic situation of Jan (Yan) is severe, but I hope I could request daily transportation expenses from the procedure fee.

This time, we carried out transplant for an elderly patient so far in China, it is the first time that I groped for the way of the operation in foreign countries.
If the work can go smoothly with Jan (Yan) in the future, it is possible for me to transfer the patients who were scheduled in China to Indonesia promptly. Please reply me as soon as possible.

Transplant operations we have arranged so far were all held in China, but since the patient of this case is at an extreme old age, we explored for the first time the possibility to offer transplant operations in a different country other than China.

When we both find ourselves working together smoothly, we can send our other patients immediately to Indonesia, even though they are planning to have their operations in China.

I would appreciate it if you would reply to me as soon as possible on this matter.

Best regards,

We have been pressing ahead with visiting China to receive a transplant for geriatric patient but, we looked in to find a way to undergo an operation in a different country, for the first time.
If we can make a problem-free deal with Mr. Yan, we can allocate our donation recipient (who are planned to receive a transplant in China,) to Indonesia at once.
I would greatly appreciate receiving a prompt response.
Thank you.
