こちらからざっくばらんに申し上げますが、費用に関してはヤン氏の手数料を入れて高くとも総額$50,000が我々が承諾出来る限界です。これが無理であれば大変残念ですが、はっきりお断りするしかございません。その場合、患者は直ちに帰国することになります。

また、ヤン氏が現在経済的に厳しい状況に有ることは小林氏から伺い十分認識しておりますが、日々の交通費等の請求については手数料の中で考えて頂ければと思います。

Frankly speaking, the total amount we can prepare including Mr. Yan’s commission fee is $50,000 at the most. If this is impractical, I’m afraid we have to turn down this offer and take the patient back immediately.

I fully understand that Mr. Yan is in a financially difficult situation (according to Mr. Kobayashi) but, please allocate the commission fee to cover the daily transportation charge.
