Please send us ALL the information we requested so we can proceed with the claim

Attached is the claim form that you will need to fill out and the instructions in order to proceed with the claim.

Once we receive all the information requested, we can then proceed with the claim. But please act on this soon as you only have 30 days since the date of shipment to file a claim or it will be denied.
