It appears that your app is repeatedly redirecting people to the Login Dialog after they decline permission requests. This means that if someone is looking at the Login Dialog for your app and clicks ‘Cancel’ rather than accepting the permissions, the Login Dialog is automatically shown again and they have no way to get out of this loop.

This is considered a violation of Facebook Platform Policy 7.5 - 'If a person declines a permission, you can prompt them again after they indicate an intent to grant you the permission.'
