I have visited the post office for what must be the last time on this issue!
Attached and appended below are the forms the U S Post Office filled out and faxed - Both of which I have done.
The post office representative also put in her name and the office address on the form as required.
Note: They inspected the shipping box, and the broken cover, and then they KEPT ALL of the parts (Box, cover, broken pieces, etc.).
I hope you can do the rest on your own as I am now finished with this project.

郵便局の代表者は彼女の名前と事務所の住所も記入してくれました。
備考：発送に使われた箱、壊れたカバーをチェックし、全てを保存しました（箱、カバー、壊れたパーツなど）。
