私はあなたの依頼への回答として、Aで使用している表をお送りします。
15年に使われた古い形式ですが、我々は現在もこれを使用しています。必要に応じて詳細項目を修正した上で、各部署へ配布し回答を記入してもらっています。これには以前使用した際の古い値が書かれたままですのでご注意下さい。これはBとCの二つが存在します。BはDの回答用、CはEの回答用です。もし想定していた表と異なる様でしたらお知らせ下さい。
またFについて、本業務に加え職場の意見を聞き、それに基づき対応策を検討しています。

As a reply to the request to you, I will send a chart which has been used for A.
Although the format is old and it was used in 2015, we still use this now. We revise the details as requested, then we distribute it to each department and ask them to fill out it with their answer. Please note that the old values which are written when the chart was used before remain as it was. It has two versions, namely, B and C. B is for D, and C is for E. Please let us know if the chart is different from what you have expected.
We are also considering measures for F based on the opinions from our employees in addition to this task.
