この申告書は控除対象配偶者や扶養親族に該当する人がいない人も提出する必要があります
この申告書は２か所以上から給与の支払を受けている場合には、そのうちの１か所にしか提出することができません
あなたに控除対象配偶者や扶養親族がなく、かつ、あなた自身が障害者、寡婦、寡夫又は勤労学生のいずれにも該当しない場合には以下の各欄に記入する必要はありません

This application is to submit in order to claim exemption for spouses, dependents and disabled persons regarding your salary.
This application should be also submitted by any person with no covered spouses or dependents.
In case of receiving the payment of the salary from more than two places, this application can be submitted to only one of these places.
If you have no covered spouses or dependents and you are none of a disabled person, a widow, a widower or a working student as well, it is not necessary to fill out each column below.
