1) Purpose of account
2) Source of funds - where and how it is derived
3) Company information - background, nature of business, address and your position
4) List of all banks that you use for you and your wife.

Kindly note that you will need to initial at the bottom of every page of the application form.
We recommend that you provide as much detail as possible to support your application, as the opening of accounts is subject to head office approval.
Please also submit copies of the following documents for each applicant:
1) Passport
2) Proof of residential address

申込用紙の全てのページの下部に署名をして頂きますようお願いします。
口座開設は本社の承認が必要な為、申請をスムーズに行う為にも、できる限りの詳細情報を提供して頂くことをお勧めします。
また申請者の方の、下記書類のコピーの提出もお願いします。
１）パスポート
２）居住地住所証明書
