先日、お電話いただいた件で
銀行のほうで私の預けた金額を誤って、兄の口座へ入金してしまったと連絡をもらいました。
兄の口座から私の口座へ移動させるのに、兄のサインを記入する必要があるそうですね。その用紙を私の自宅に送ってくれたとのことですが、いまだに届いていません。
いつ届きますか?

The other day, regarding the matter that you called me about, the bank made a mistake about the sum of money supposed to be given to me. I received notification that this sum of money has been inputted into my elder brother's account.
For the money to be transferred from my brother's account to mine, it seems that my brother's signature is required. Although the necessary document was supposed to be delivered to my home, I have not received it even now.
When will this document arrive?
