For the credit card I will need the 16 digit card number, expiration date, 3 digit security code from the back of the card, and who’s name appears on the card.

For the shipping address I would like to confirm that with you. I am having a hard time with the fax copy and I want to make sure it is right. Please type the address in your reply.
