・会員番号/ご住所/お名前/日中連絡の取れる電話番号/チケット返送枚数
(For example, if you want a refund for 2 tickets out of 4 tickets purchased, send back 2 tickets.)
※If you return more than one tickets, the person who purchased needs to send all of the tickets together.
※ You cannot get a refund for the tickets purchased unless you purchased from Koda gumi／playroom as an exclusive advance offer.
