区分欄の上端と下端の文字が途切れております。また，文字の上にチェックボックスが重なっており
区分が把握できなくなっております。さらに右下のサイン欄の記載が漏れております。

・右下のManufacturer signature欄に記載のサインは製造者（TAB Co Ltd）のもので間違いないでしょうか。
恐れ入りますが，どなたのサインかが判読できません。製造者の捺印であることが分かるような体裁でいただけますでしょうか。

The characters at the top and bottom of the classification column are cut off. The checkboxes also overlap the characters.
I can no longer grasp the classification. The signature column on the bottom right is also missing.

・Are you sure that the signature in the 'Manufacturer Signature' column at the bottom right is that of the manufacturer (TAB Co Ltd)?
Unfortunately I cannot decipher whose signature this belongs to - please could you format it so that I can see that it has the manufacturer's seal?
